2008-2009 School Year
The 2008-2009 school year is in full swing and our classes are full. You are welcome to apply for any openings that may occurr throughout the school year.
To apply for grades K-8, complete the waiting list application below and bring to the school office with your child's birth certificate and any religious documents such as a baptismal certificate. You are also required to bring your child's report card from this year and last year as well as any standardized test results for the current and previous year. (As an example, some schools use the Iowa (ITBS), the Stanford 9 or the AIMS.) The kindergarten and first grade application fee is $50 and the 2-8 grade application fee is $25. Your application is not considered complete and will not be reviewed or added to any applicable waiting lists until all the required documents and fees are submitted to the office.

2009-2010 School Year
Kindergarten applications for the 2009-2010 school year are now available. We will accept completed applications in the school office starting on November 3, 2008. All kindergarten applicants must by 5 years of age prior to September 1, 2009. Kindergarten applicants will be Gesell tested in January and February of 2009. When completing your kindergarten application, include the completed application form, birth certificate and Baptism certificate.
If you have more than one child applying to St. Francis Xavier, please feel free to use the Sibling Addendum for each additional child.
The Parish Verification Form should be completed with each application. It can be found below in the area labeled 2009-2010 Application Packet. Please complete Section A and be sure to sign the form and turn into the school office with your application.
If you are a member of another parish, please complete Section A and Section C and return to the school office.
