Kindergarten applications for
2011-2012
will be accepted on Monday,
November 4, 2010
in the school office.
A $75 application fee, birth certificate, application,
and baptism certificate (if applicable)
are required documentation.

Be sure to attend
the Kindergarten
OPEN HOUSE
on Sunday,
October 31, 2010
at 10:00 a.m.
in the school cafeteria.
There will be
tours and a presentation in the library at 11:00 a.m. |
Applications for the 2010-2011 School Year
We currently have openings in:
Kindergarten
3rd Grade
All other grades are completely full and we have a waiting list.

All kindergarten students must be 5 years of age by August 31, 2010.
The application fee is $25 and the Gesell screening fee is $50.
(All kindergarten students are Gesell screened. For more information on the Gesell Assessment Screening, go to their website by CLICKING HERE.)
Application for grade THREE for the 2010-2011 school year.

Applications are not accepted without all the documentation listed
on the front of the application and the $25 application fee.
The required documentation includes:
~ birth certificate
~ sacrament documents
~ 2 years of report cards
~ 2 years of standardized test results (such as AIMS, ITBS, Stanford 9 etc.)
The Parish Verification Form should be completed with each application. It can be found below in the area labeled 2010-2011 Application Packet. Please complete Section A and be sure to sign the form and turn into the school office with your application.
If you are a member of another parish, please complete Section A and Section C and return to the school office.
